Delta RFBC Technical Assistance (TA) Provider Reporting Instructions - Please Read Carefully

Thank you for supporting the Delta Regional Food Business Center (Delta RFBC) initiative.

Frequency: To help us track and improve the impact of our work, please complete and submit the TA Provider Reporting Tool monthly (3rd of each month) with invoice submission.

Monthly Submission

Send your completed TA Data Collection Tool via Submittable. Email the TA Invoice to Joseph Garcia at jagarcia@warehouses4good.org.

In Submittable, attach relevant materials for the TA Data Collection Tool as applicable, such as:

  • TA session logs or summaries
  • Sample business plans
  • Financial/market analysis tools
  • Other supporting documentation

Submittable integrates with Nutshell or Salesforce if you already use those systems. Regardless of platform, all PPR-required data must be reported. A reporting portal within Submittable offers the DRFBC TA Provider Data Collection tool and an invoice template for partners without tracking systems.

Purpose of This Report

The TA Provider Data Collection Tool helps us:

· Collect consistent, accurate data on the technical assistance (TA) provided

· Evaluate TA effectiveness across topics, regions, and recipient types

· Inform USDA and regional reporting requirements

Estimated Completion Time: 30–45 Minutes

The tool includes both structured and narrative questions. Please have the following ready:

  • Provider and organization contact info
  • TA types and formats (e.g., 1:1, workshops)
  • Topics covered (e.g., financial training, marketing, supply chains)
  • Participant lists with number of participants, names and addresses
  • TA hours (pre-award, post-award, or unrelated to Business Builder)
  • Outcomes (e.g., business plans, product integration, partnerships, financial improvements, etc.)
  • Quotes or testimonials for USDA impact stories

If you experience any technical issues with submitting the data collection tool please contact Laura Louden: laura.louden@mbakerintl.com

If you have questions about the form content, please contact Danita Rodriguez at DRAResources08@gmail.com.

Final Notes

All form fields support broader data analysis and storytelling. Accuracy and completeness are vital—they help shape how we demonstrate our collective impact.

Thank you for your time and dedication. Your input is essential to documenting the transformation of regional food systems through the Delta RFBC.
